Healthcare Continuum Group, LLC Is a group of clinicians (registered nurses) and other industry specialists who have varied experience in a multitude of settings. We have spent many years at the bedside of patients caring for patients and their families and understand the needs of patients and families. We have advocated for patient and families, facilitated communication between patients/families and other health care providers, coordinated services at all levels (in and out of the hospital) and have represented and obtained authorization for services from insurance companies.

We have extensive experience and knowledge of the health care and insurance industry, understand the different levels of care inside and outside the hospital and have had experience advocating on the behalf of numerous patients to ensure they receive the highest level of care.

Our Mission is to improve health care delivery for Americans across the country, by providing a concierge of products and services. Purpose; To improve patient and organizational outcomes by enhancing processes and communication between healthcare providers, patients, families and caregivers.
